Breakthrough partners with those affected by poverty to build connections, develop skills, and open doors of opportunity. Breakthrough has a hyper-local focus within a 40-block zone in the East Garfield Park community on the west side of Chicago, just a few miles west of downtown.

Men's Center Fresh Market Women's Center FamilyPlex Violence Prevention Center

Men's Center

Est. 2008

The Breakthrough Men’s Center provides a transitional housing program that meets the housing and basic needs of men experiencing homelessness or crisis. Wraparound services include job training, case management, medical care, mental health care, and substance abuse treatment. This location also houses the permanent supportive housing services for individuals and families experiencing homelessness.

Fresh Market

Est. 2014

The Fresh Market is a client-choice food pantry that provides fresh, high-quality groceries and additional supplies to more than 10,000 residents of East Garfield Park each year. The building was opened in 2014, but the Fresh Market has been a core part of Breakthrough for nearly two decades.

Women's Center

Est. 2000

Breakthrough’s first building in East Garfield Park, the Breakthrough Women’s Center, provides a transitional housing program that meets the housing and basic needs of women experiencing homelessness or crisis. Wraparound services include job training, case management, medical care, mental health care, substance abuse treatment, and the women’s clothing closet.

FamilyPlex

Est. 2015

Born out of the vision of local youth, the FamilyPlex is home to the Breakthrough’s youth and family programs,
including early childhood education, afterschool programs, arts and sports initiatives, and opportunities for high school students. Additionally, the FamilyPlex offers the Bridge Cafe & Catering, the Fitness Center, and a medical clinic in partnership with the Lawndale Christian Health Center.

Violence Prevention Center

Est. 2019

After violence impacted the corner of Homan and Walnut Avenues in 2018, Breakthrough partnered with a local landlord to renovate and occupy a storefront. Now, the space houses Breakthrough’s violence prevention and intervention services for the East Garfield Park community as part of Communities Partning 4 Peace.
