Supporting individuals experiencing homelessness in securing and retaining housing and building life skills

Ages 18+

Breakthrough’s Transitional Housing Program is a shelter program where individuals stay for an extended period of time while they work toward goals around self-sufficiency. This is a housing-first, trauma-informed program that provides a dedicated bed, care coordination, and housing retention support. Through strong partnership and connections to resources, together we will address barriers such as employment, substance abuse, mental and medical health needs, and legal aid.

Potential participants must be referred through the City of Chicago. To start the referral process, please visit a Coordinated Entry System Access Point or contact the Coordinated Entry hotline at 312-361-1707, Monday–Friday 8:30am-4pm.
